Here is the version of this that keeps operations leads up at night. A figure gets pulled from one system and written into another — a quantity, a price, a customer record, a status — and somewhere in that hop it lands wrong. Not dramatically wrong. Quietly wrong. It flows into a report, then into a decision, then into the next month's numbers, and by the time anyone notices, the trail has gone cold. You can't reconstruct what happened because nothing recorded what the old value was, what changed it, or when. The damage isn't the one bad edit. Nothing irreversible happens silently. On top of that, the Approve-Gate watches for anything outside the normal pattern: a value that jumps out of its usual range, a field changing that almost never changes, a batch that's larger than usual. When something trips it, the flow stops and waits for a person to look before it writes. The boring, in-pattern 95% flows through untouched; the suspicious few wait for a human.

The rule I never break: the automation does not get to write something it can't show you and can't take back. If a change is risky or unusual, the Approve-Gate holds it for a human. If it's routine, it still leaves a record you can replay later.

The point isn't that the AI never makes a mistake. The point is that when it does, you can see it, prove it, and reverse it — instead of discovering it three weeks too late with no way to explain what happened.
